Kaiser Permanente is seeking a compassionate Social Worker to join our Healthcare & Medical Services team. In this role, you will provide psychosocial assessments, counseling, and care coordination for diverse patient populations across inpatient and outpatient settings. You will collaborate closely with interdisciplinary teams to address social determinants of health, support safe discharges, and connect patients with community resources. Provide psychosocial assessments, counseling, and care coordination for patients and families across the continuum of care
Develop and implement individualized care plans in collaboration with physicians, nurses, and interdisciplinary healthcare teams
Advocate for patient rights, access to community resources, and continuity of care post-discharge
Facilitate crisis intervention, safety planning, and referrals to mental health and social service agencies
Document all clinical interactions in accordance with regulatory, ethical, and Kaiser Permanente standards
Participate in case conferences, quality improvement initiatives, and interdisciplinary rounds
Educate patients and caregivers on treatment options, coping strategies, and community supports
Maintain current knowledge of healthcare regulations, payer requirements, and community resource networks
Support population health initiatives and outcomes tracking within Healthcare & Medical Services
Uphold ethical social work practice, cultural humility, and trauma‑informed care principles
