Company Overview
The mission of The Coordinating Center is to partner with families, children and adults with special health care needs and disabilities and those who support them in the community to achieve their aspirations for health, inclusion, independence, dignity and quality of life.
The Coordinating Center is a non profit organization (501 c 3) that coordinates the services that people with complex medical needs and disabilities require to stay in their own homes and communities. We also coordinate the steps that are necessary for people to move from nursing homes and other institutions to homes of their choosing. We do this by working cooperatively with the people we serve as well as family members, housing agencies, providers of necessary services, and community resources and funders in a family/person centered dynamic. A statewide organization, The Center is staffed by specialists in nursing, social work, special education and rehabilitation, infants and toddlers services, housing and community resource development. The people we are privileged to serve are those with the most challenging needs who meet the criteria to participate in one of the following programs:
The Model Waiver Program for children with complex medical needs and disabilities who are not otherwise eligible for Medicaid but who require significant help at home.
The REM (Rare and Expensive Case Management) Program for adults and children with specific diagnoses and disabilities who have Medicaid eligibility.
The Autism Waiver Program for children with autism spectrum disorders who need assistance to thrive at home.
The Living at Home Waiver Program for adults with disabilities living in nursing facilities that need assistance to transition to homes of their own in the community; recognized for success in implementing comprehensive transition and locating affordable, accessible homes in the community.
Infants and Toddlers Service Coordination provided to children requiring early intervention who are participating in REM or the Model Waiver.
The Amerigroup Care Coordination Program is an alliance with this managed care organization to provide targeted case management among participating individuals with chronic illnesses; designed to avoid unnecessary hospitalizations.
The Coordinating Center also provides comprehensive life care planning, expert testimony and structured settlement care management and eldercare care coordination through Medical Legal Services. These services are designed to provide expert planning relative to people with catastrophic care needs over their life span.
ACCESS Group for Consultative Services offers a diverse complement of technical assistance, training, and consultation around the important issues of person centered approaches, quality and systems design associated with programming for people with special needs and disabilities in the community. New projects are implemented through the ACCESS Group.
