ACM Care is a local, nonprofit agency dedicated to serving the needs of local senior citizens and their families.
Our Mission statement:
To improve the quality of life for frail, elderly and disabled people living in the community through case management and support services, regardless of their ability to pay.
Company History
ACM Care (Autonomous Case Management) was founded in 1999 to meet the needs of community--dwelling senior citizens who want to remain living in their homes.
Since inception, ACM Care has expanded to provide additional needed services incuding nursing home and hospital screenings, caregiver support, money management, medical/medication management, bill paying, and personal assitant services.
