About LifeLong Medical Care: LifeLong provides high-quality health and social services to people of all ages; creates models of care for the elderly, people with disabilities, and families; and advocates for continuous improvements in the health of our communities. LifeLong Medical Care operates seven community clinics in Alameda and Contra Costa counties, a Supportive Housing Program, and two Adult Day Health Centers. We currently serve over 23,000 patients in 120,000+ visits annually. Our services and programs continue to expand with the needs of the communities we serve.
